Plan 4 Life are a boutique service provider specialising in disability accommodation, working with Providers and Participants throughout Victoria.
STA (RESPITE)
We arrange tailor-made respite breaks for individuals and groups throughout Australia.
SUPPORT SERVICES
We provide a range of support services to clients who are NDIS Participants within the Melbourne area
Care workers
We provide opportunities for support workers to contract with us for placement for support work.
Support Services
Social and Community Participation
We provide you with support for everyday activities when you need to leave home such as shopping, attending community events, recreational activities, attending appointments and accessing other programs.
Support Coordination
Our Support Coordination team assist participants in navigating the disability service system by providing information, advice and support when decisions need to be made.
We monitor progress, resolving any further arising issues and suggesting an adjustment of ongoing support when required.
Our team has experience in crisis resolution and will work to increase individual’s resilience so they can reach their potential.
In Home Care
We provide you with support for daily living such as bathing, meal preparations, cleaning, gardening and other housekeeping services.
Respite Break Opportunities
